Returning Student Registration Form
for the fall 2025 session


(Tuesday 9 September - Thursday 11 December)

This form is only for students who are attending/have attended one or more sessions.  One form per student.  All Belt levels (White to Black) need to register.

New students who want to join the session need to complete the
New Student Registration Form.

     Submission Deadline 
    Friday 22 August 2025
