FAMILY INFORMATION. For students under 19 years old (i.e. minors), enter name(s) and phone number(s) of parent(s)/guardian(s) as applicable.

I WISH TO REGISTER FOR THE FOLLOWING CLASS:

Children 7 - 10 year old class (Tues and Thurs 5:30 pm - 6:30 pm). Full session fee: $125.00.

Youths (11+)/adults class (Tues and Thurs 6:45 pm - 8:00 pm). Full session fee: $160.00.

Family discount available. See Q&A 3.9.

NOTE

New students must reach the minimum age by the end of the starting month to join the class in their age group.

I WISH TO REGISTER FOR THE FOLLOWING SESSION:

Fall 2025 session (runs until Thursday 11 December). Full session is 14 weeks. Fee will be pro-rated based on starting date.

Winter/spring 2026 session (runs from Tuesday 3 February to Thursday 28 May). Full session is 15 weeks. Fee will be pro-rated based on starting date.

POSTING OF PHOTOGRAPHS/VIDEOS ON THE SHUSWAP DOJO WEBSITE, THE CEDAR HEIGHTS COMMUNITY ASSOCIATION WEBSITE AND OTHER ONLINE OR PRINT MEDIA. I authorize the Shuswap Dojo to post individual or group photographs/videos (taken during classes) of the student named on this registration form.

SINGLE INVOICE. Will be issued upon receipt of registration form and due two weeks later.

TWO INVOICES. First invoice (50%) will be issued upon receipt of registration form and due two weeks later. Second invoice (remaining 50%) will then be issued and due four weeks later (subject to time remaining in session).

New students and parent(s), as applicable, are asked to be at the Dojo at 5:00 pm (for the children's class) or 6:30 pm (for the youths/adults class) the first day they attend a class. This time is needed to

Meet the new students/parent(s).
Be fitted for a uniform.
Sign the release of liability form (must be signed by an adult (19+)).
Have your picture taken for our attendance records.
Provide you with your Student Record of Promotions booklet.
Allow us to answer any questions you may have.

PAYMENT

No payment is required when submitting the New Student Registration Form. You will be invoiced after receipt of the form.
